Con Amor y Dignidad es un programa para facilitar información y abogar por un trato digno a las personas latinas mayores de edad que enfrentan enfermedades graves, así como a sus cuidadores familiares. Contamos con un equipo de promotoras líderes comunitarias, expertas en la navegación de programas de salud y servicios sociales, que brindan apoyo a cuidadores y a personas con condiciones crónicas, enfermedades severas o diagnósticos de etapa terminal.

Our advocates are here to provide information about resources and support, helping to develop connections that allow access to these services. We promote well-being and encourage open communication among families about care and end-of-life decisions. We also work to increase awareness of Advance Care Planning, ensuring that both caregivers and patients receive the support and resources they need in their community.

Our goal is to provide dignified care in managing conditions such as heart disease, stroke, cancer, serious complications of type 2 diabetes, Alzheimer’s, and more, at every stage of this life.

The program is completely free and is offered in Spanish and English, always with a warm and understanding approach. We are here to support you!
